Piezoelectric coupling not working

Hi,
I am trying to solve a model where a fluid channel is attached on the top of the piezoelectric substrate. The piezoelectric substrate is excited by applying a sinusoidal voltage. The coupling is done by passing the velocity of the piezoelectric material to the Navier stokes module as a boundary condition at the interface. However, I get an error saying inconsistent initial values. My model is attached. can someone please have a look and help me with this.
