"May have reached a singularity" ERROR

I've been working on the Upper-Convected Maxwell model. I've modeled it in 3D but have been getting the singularity error after repeated attempts. Can someone please tell me if there is something wrong with any of the BC's that I have used because I've gone through them infinite times but am still getting the error. Please Find attached the model
