Issues with swept mesh

Hi

I am having issues with the Swept meshing command. I tried different things listed in the reference manual but could not complete without errors. It could be a small mistake I might be doing. Always I get "failed to create swept mesh for domain" and "each destination face must uniquely correspond to one or more source faces".

Could you check the uploaded mph file and let me know what was the mistake I am making.
