Electromagnetic Singular Matrix Error

Whenever I run the 2D simulation below for a tapered fiber coupled to a rectangular waveguide, I end up with an error that says "Detail: There are 1 void equations (empty rows in matrix) for the variable comp1.ewfd.S1x. at coordinates: (0,0),...". I have my MPH attached, any suggestions as to how I'm setting my boundary conditions incorrectly?
