Coefficient form PDE with two dependent variables

Hi, I need to model this equation (shown in the image attached) in COMSOL. For which coefficient form PDE sounds good.
Two dependant variables: s,Ф
If I select number of dependant variables = 2 from "Dependant variable section" of coefficient form PDE. Then the value of coefficient (ea, da, c etc) shows four spaces how to put in the coefficient values then?
Any suggestions for modelling this equation please?
Can anyone guide in this regard? Thanks in advance.
